Waiver and Release Liability and Make up policy
Tennis Kiddo’s Liability Waiver
In consideration of being allowed to participate in the Tennis Kiddo’s program, I, parent/guardian of the participant, hereby acknowledge and agree to the following terms and conditions:
Assumption of Risk: I understand and acknowledge that participating in Tennis Kiddo’s activities exposes my child to inherent risks, including accidents, injuries, and illness. This includes but is ot limited to, falls, contact with other participants, equipment, or tennis balls. These risks are known and appreciated by me/us.
Full Responsibility: I accept full responsibility for my child’s usage of any equipment, facility privileges, or services owned and operated by Tennis Kiddo, at our own risk. I/we shall not hold Tennis Kiddo, its staff, instructors, representatives, or property owners liable for any loss, claim, injury, or liability incurred during voluntary participation in Tennis Kiddo’s programs and activities.
Communication of Concerns: I acknowledge our responsibility to communicate any physical or psychological concerns that may affect my child’s articipation in any activity. We further acknowledge that my child is physically fit and mentally capable of engaging in the chosen physical activities.
Release and Waiver of Liability: I, on behalf of my child and anyone acting on our behalf, agree to hold harmless, waive, and release Tennis Kiddo, its officers, agents, instructors, organizers, representatives, successors, and any property owners from any responsibility, liabilities, demands, losses, injuries, property damage, or claims of any kind arising from participation in Tennis Kiddo’s activities, events, or use of facilities, equipment, or property.
Medical Responsibility: I attest that we have full knowledge of the inherent risks involved in Tennis Kiddo’s activities and assume these risks. We agree to assume and pay for any medical expenses and emergency expenses in the event of accident, illness, or other incapacity.
Medical Information: I agree to provide appropriate medical information to Tennis Kiddo if required. In the case of an accident or injury to my child, and if an emergency contact person cannot be reached, we grant Tennis Kiddo permission to obtain necessary medical attention for which we will be financially responsible.
Photography and Publicity: I grant permission for Tennis Kiddo to use photographs, videos, and other media of the participant for promotional and educational purposes. Any such material may be used in marketing materials, social media, and the Tennis Kiddo’s website.
Property Loss: I understand and agree that Tennis Kiddo is not responsible for property that may be lost, stolen, or damaged while on its premises. I/we have read and understand this liability waiver and agree to its terms.
Make up policy
We understand that sometimes unforeseen circumstances may arise, and you may need to reschedule your
class. Here is our make-up policy for rescheduling:
● Availability: Make-up classes are only available if there is space in another class, and they are subject
to class availability.
● Advance Notice: To request a make-up class, you must notify us at least 24 hours prior to your
originally scheduled session by sending an email to tenniskiddonyc@gmail.com. This advance notice
allows us to allocate the spot to another student.
● Advance Notice: Each student is eligible for a maximum of one reschedule request, considering the
limited availability of make-up classes.
● No Transfer to Future Sessions: Please note that make-up classes cannot be transferred to future
sessions. They must be used within the current session.
● No Credits Offered: We do not offer credits or refunds for missed classes. Make-up classes are
provided as a courtesy when available.
We aim to accommodate your reschedule requests to the best of our ability, but please understand that our
make-up class availability is limited and may not be guaranteed. If you have any questions or wish to request a
